
Who We Are
YoloCares is a non-profit, community-based organization. It was run by volunteers until becoming Medicare and Medi-Cal certified in 1995. Since then, YoloCares has developed a highly trained interdisciplinary team including a physician, a nurse practitioner, an all-RN nursing staff, Masters-trained Social Workers, Spiritual Care Counselors, Certified Nursing Assistants, Bereavement Specialist, and a large base of volunteers specially trained in end of life care. All these experts work together to develop an individualized care plan for each patient, focusing on the whole person and their family’s needs.
Bereavement Volunteers work with our Bereavement Department, reaching out to patients’ families after a patient has passed away. Bereavement volunteers call caregivers and loved ones to offer information about our individual grief and group support programs. We offer a variety of drop-in and multi-week support groups for adults, and we offer our Stepping Stones program for children and teens.
Our Bereavement services are available free of charge to the entire community, whether or not your loved one was a patient of YoloCares. Bereavement specialists and trained volunteers facilitate groups and family meetings and provide individual support sessions, home visits, and telephone contact as needed. Referrals to additional support resources are made when appropriate.
What We Do
Support grieving families through phone calls, assistance with memorial services, or ongoing support groups. The bereavement support volunteer provides follow-up information, emotional support, and companionship to clients under the supervision of the Volunteer Manager and/or Bereavement Support Manager. Bereavement support services are available to anyone in the community, regardless of whether or not their loved ones received care from YoloCares.
