We have an 57 year old male patient residing at home in Davis diagnosed with ALS and has MaID. Patient is alert and oriented, uses a wheelchair at times and needs assistance. There are two cats in the home. Locked guns in home.
Wife & Daughter are requesting a doula for emotional support for themselves, potential engagement with patient and his acceptance, planning for EOL. Family has been working with a doula who was out-of-state and feel they need someone in-person for the remainder of the journey. Doula will be of emotional support to wife, daughter, and Son (out-of-state). Assist in coordination of EOL journey and peperwork.
